About Us
AVENUES TO COMMUNITY, INC. by contract with Dane County, provides on-going service coordination to adults with developmental disabilities who reside in Dane County. Avenues supports adults who live with family, in supported living arrangements, or in Adult Family Homes. Avenues has been serving people with developmental disabilities since 1984.
Supporting adults with developmental disabilities and traumatic brain injuries in realizing their dreams and desires by helping them find the supports and services they need to live and work in the community.
Avenues staff, referred to as Support Brokers, provide case management and service coordination to adults who live in a variety of residential settings including supported living arrangements, families' homes and Adult Family Homes. Service coordination involves developing, monitoring and implementing an individual's support plan so that he/she can receive the services needed to live and work in the community. Service coordination also involves maintaining a person's eligibility for available funding sources and exploring community supports.
Avenues staff focus on individual choice, building relationships and community involvement for each person. Each staff has an expertise including: Person Centered Planning Behavioral and Communication Issues Law Enforcement and the Judicial System Community Building Advocacy Aging Mental Health Multicultural Issues
Avenues Support Brokers conduct annual meetings with the individual, their guardian and support team to discuss the person's preferences and service needs. Once the person's service goals are identified, the Support Broker provides on-going case management and service coordination. |
